Student's Good Health Helps Classroom Performance
It's a well-known fact; your child's health will influence his or her progress in school. That's why our district recommends a complete physical examination before children start kindergarten, and again before entering 7th grade.
An examination helps staff plan an education program suited to each pupil, and may identify children in need of special medical attention. It also assists in the prevention and early treatment of disease.
For students who participate in athletics, a physical exam is required at grades 7 and 10. Required forms are available at middle and high school main offices.

Children entering kindergarten are required to have received:
Students in grade 7 are required to receive boosters so that they have had:
Students in grades 8 through 12 are required to receive boosters so that they have had:
Parents may obtain a legal exemption from the school immunization law for medical reasons or conscientiously held beliefs. Please contact your school nurse if you need information about an exemption or about low-cost immunization or physical examination services.
Also, students will receive an annual health history form when they return to school. Please return the completed form to your child's school.
