Forms: Before a student is allowed to practice or play in an East High sport, she/he must have the following forms on file in the East Activities Office: (1) Official ISD 709 Physical examination Form, (2) MSHSL Form – signed by the student and parent, (3) ISD 709 Academic/Chemical Form – signed by the student and parent, (4) East Athletic Information card – (Yellow) Physical examinations obtained in either the 7th, 8th, or 9th grades are valid for participation through the 9th grade. Any student who wants to participate in a sport in the 10th, 11th, and 12th grade must have a physical examination form prior to participating in the first sport. Physicals obtained in grade 10 - 12 are valid for 3 years. Athletic Eligibility Checklist (must be completed by all athletes) (If you cannot check all 5 items, see your athletic/activities director) _______ 1.Physical exam must be “NEW” for Grades 7-9 and 10-12. After 6/1. _______ 2.Have not transferred schools. _______ 3.Will not participate in more than six (6) seasons in any sport in grades 7-12. _______ 4.Have not accepted cash in any amount or merchandise valued at more than $100 for participating in a sport. _______ 5.Have not and will not compete in non-school events in my sport after reporting for the school team. |
